Over time, your body and aesthetic goals can change, and so can your breast implants. Whether due to aging, complications, or simply wanting a new look, breast implant exchange is a common and highly customizable procedure. At Cosmetic Surgery Associates in Washington, DC, our board-certified plastic surgeons help patients refresh their appearance with natural-looking, confidence-boosting results.

What Is a Breast Implant Exchange?

A breast implant exchange is a surgical procedure that replaces old breast implants with new ones. Patients choose to undergo this procedure for a variety of reasons, including changes in personal preference, addressing implant-related complications, or improving the appearance of the breasts after aging, weight fluctuations, or pregnancy.

This surgery is often part of regular implant maintenance and offers the chance to update implant type, size, and placement using modern techniques and materials.

3 Signs It’s Time for a Breast Implant Exchange

1. Noticeable Changes in Breast Appearance
If your breasts look different than they once did, such as sagging, asymmetry, or loss of volume, it could be time to consider an exchange. These changes may result from aging, pregnancy, or weight changes and can affect the overall shape and symmetry of your breasts.

An implant exchange can restore a more lifted, youthful, and balanced look, sometimes combined with a breast lift for optimal results.

2. Implant Complications or Discomfort
Implants are not lifetime devices, and issues can arise over time. Common reasons for implant exchange include:

  • Capsular contracture (scar tissue hardening around the implant)
  • Implant rupture or leakage
  • Shifting or displacement

With today’s advanced implant materials and surgical techniques, many patients experience better outcomes and improved comfort after a replacement.

3. Desire for a Different Look
Aesthetic preferences can evolve. Some patients decide to change their implant size, switch from saline to silicone (or vice versa), or choose implants with a more natural shape or feel.

During a consultation, our surgeons help you select the implant type that best aligns with your current lifestyle and appearance goals.

Benefits of a Breast Implant Exchange

Choosing to replace your implants offers several advantages:

  • Improved comfort with newer, more advanced implant materials
  • Enhanced aesthetics tailored to your current preferences
  • Addressing both functional and cosmetic concerns in a single procedure
  • Peace of mind from correcting complications or outdated implants

What to Expect During the Process

At Cosmetic Surgery Associates, the breast implant exchange process begins with a thorough consultation, during which your surgeon will discuss your aesthetic goals, evaluate your current implants, and develop a personalized surgical plan tailored to your needs. The procedure itself is performed in our private, fully accredited surgical suite, where patient safety and surgical precision are top priorities.

Breast implants remain a popular form of cosmetic surgery in North America. Most patients in Bethesda, Maryland, understand their implants will not last forever, with a replacement needed in the future. The proper care of breast implants helps keep patients healthy and avoid the chances of implant failure. By remaining healthy and taking care of your body, breast implants do not need to be replaced as often as some patients believe.

When Should You Replace Your Implants?

The patients of Dr. Jabs, Dr. Magge, and Dr. Richards understand how they must care for their breast implants. A common misconception about breast implants is that they need to be replaced every 10 to 20 years. The FDA has approved the use of breast implants for between 10 and 20 years, but this does not mean that the implant expires once it has reached this age. Only one in five patients need to replace their implants after ten years, with the majority seeing a prolonged period of use. Most patients only have to replace their breast implants once or twice in their lifetime.

When Should You Seek Help With Your Implants?

Since it can be difficult to diagnose these problems on your own, it’s often a good idea to schedule a consultation at Cosmetic Surgery Associates before the approved life of your implants is over. These are some reasons why your breast implants may need to be removed and/or replaced:

Hardening of Breast Implants

One common issue that breast augmentation patients face is the hardening of their breasts. This can be caused by capsular contracture, which is the hardening of scar tissue around the implant. Hard lumps in the breast, caused by calcium deposits around the implant, can also develop as a result of breast implants. Either of these conditions can be cause for removing or replacing your breast implants.

Ruptured Breast Implants

One of the rarer implications of needing a breast implant replacement is the rupturing of the breast implant. Breast implant rupture occurs in around two to twelve percent of breast implant recipients. Silicone implants can rupture completely, while saline implants can begin leaking a non-toxic solution. Most patients feel few symptoms of ruptured or leaking implants until their breasts become uneven or misshapen.

Pregnancy and Weight Gain

Certain bodily changes can lead to your breast implants needing to be replaced. During pregnancy, the breasts will first swell as you breastfeed and then go down in size, causing the position of the implants to shift. Similarly, if you have experienced significant weight loss or weight gain, the redistribution of fat in your body can also cause your breast implants to move.

Schedule a Consultation

If you are interested in exploring the option of breast implant replacement, schedule a consultation with Dr. Jabs, Dr. Magge, and Dr. Richards at Cosmetic Surgery Associates. To get started, fill out our online contact form today.

Without a doubt, breast augmentation ranks as one of the most popular cosmetic surgery options for women. If you are unhappy with the size or shape of your breasts, getting breast augmentation surgery may be the best thing for you.

Of course, you want your breasts to feel as natural as possible after this procedure. As with any cosmetic surgery treatment, it takes time before you start to feel normal and natural again. After you get your breast augmentation, you may notice that your breasts feel somewhat hard to the touch and that they sit higher on your chest than you’d like. This is totally normal and will pass over time. As you begin to heal, your new breasts will go from feeling hard to gradually settling and softening so that they look and feel more like natural breast tissue.

When Will My Breast Implants Soften?

After your breast augmentation surgery, you may feel concerned to find that your breasts feel too hard and seem to sit too high on your chest. However, you should take some assurance in the fact that this is not permanent. Your final results will be much better. A variety of factors can come into play that determine how long it might take for your new breasts to soften. Breast implant softening, also known as “fluffing,” can take anywhere from eight weeks to six months. If your breasts don’t feel soft by the six-month mark, you should make an appointment to see your plastic surgeon at Cosmetic Surgery Associates.

After your surgery, it’s normal to experience swelling. This is one of the biggest contributors to your new breasts feeling firm and too high. Over time, your breast implants will drop into proper position and soften as the swelling goes down. At that point, your new breasts will begin to feel and look more natural. However, the larger your implants, the longer this process takes.

Sometimes, a woman will notice that one breast implant softens and the other one remains hard, even beyond six months after the procedure. This can be a sign of capsular contraction, a complication that occurs when there is too much scar tissue after breast augmentation surgery. Due to this potential risk, if your breasts still feel hard or one feels hard while the other has softened, you need to follow up with your plastic surgeon and schedule an appointment to have the procedure revised.

How Can the Soften Process Be Expedited?

You can do your part to expedite the softening process of your breast implants. Your plastic surgeon may recommend daily breast massages once you are cleared to do so. You can massage your implants by gently pressing them inward, outward, and up and down multiple times per day. This massage process can help your breast pocket to expand, which allows your implants to settle in their proper position. There is an additional benefit as it can also help to prevent capsular contraction from developing.

What’s the difference between saline and silicone breast implants?

The primary difference between saline and silicone implants is the contents of the silicone shell insert. Saline implants contain salt water, while silicone implants are filled with a more viscous silicone gel. During your consultation at Cosmetic Surgery Associates, a board-certified surgeon can help you decide the right choice for you.

What are the risks of breast implants?

Some of the risks associated with breast implants can fade over time, such as pain or changes in sensitivity in the breast. Others might require another surgery to fix, such as an infection, a ruptured implant, or the formation of scar tissue (capsular contracture) which can make the breast firm instead of soft.

What happens if an implant ruptures?

Ruptures happen for a variety of reasons. Whatever the cause of the rupture the implant should be removed, though a new implant can often be placed during the same surgery. When a saline implant is ruptured, the salt water is absorbed by your body with no ill effects. The gel in a silicone implant is more viscous than water and remains encased in the tissue that forms around the breast implant. In either case, a ruptured implant should have no long-term ill effects on your health, however, ruptured implants do require removal.

What factors might I consider before getting breast implants?

During your consultation at Cosmetic Surgery Associates, a surgeon will speak with you about the factors that you might weigh as you consider breast augmentation. Breast implants are not a remedy for sagging breasts. You might consider speaking with your surgeon about a breast lift as well as augmentation during your consultation if you would like to alter the shape and position of your breasts as well as the size.

Various factors can change the appearance of your breasts after augmentation. These include capsular contracture (scar formation around the breast implant) and, less frequently, implant migration. In the unusual event that these occur, Cosmetic Surgery Associates is dedicated to helping you maintain your cosmetic goals and sometimes additional surgery may be necessary.

Note that outside of the context of breast reconstruction or similar procedures, breast implants are not covered by health insurance as they are not medically necessary. An experienced surgeon at Cosmetic Surgery Associates can answer all your questions about the details of these factors as they relate to you.

What’s next?

We understand that the decision to undergo breast augmentation is highly personal and requires a lot of information. If you are interested in learning more about breast augmentation, contact Cosmetic Surgery Associates today to schedule a consultation with an experienced surgeon to discuss your individual needs.

Breast augmentation is the most popular cosmetic surgery from year to year. With such popularity, it would seem like it’s easy to choose to go through with a breast augmentation, but there are a lot of things that should go into your decision to get breast implants. The best way to make an informed decision is with the help of a board-certified plastic surgeon like Dr. Jabs, Dr. Magge, or Dr. Richards, but here are some factors to be prepared to discuss during your consultation if you’re opting for breast enhancement.

You may need a breast lift

If you’re hoping to correct sagging or deflated breasts with breast surgery, then you’ll likely need to add another step to your augmentation. This is called a breast lift, or mastopexy, and it specifically addresses breasts that deflate or sag because of things like pregnancy or breastfeeding. It does this by lifting the breast tissue to a higher, more upright position before excising any extra skin. This procedure, sometimes called a breast augmentation lift, can produce excellent, rejuvenating results.

Your implants will require some monitoring

Implants can come with some extra steps after your surgery and recovery are complete. The reality is that they can rupture or succumb to complications like capsular contracture, leading to the need for another surgery. For this reason, you’ll need to be familiar with them and monitor them for any problems. You should also be sure to keep a good relationship with your plastic surgeon so that they can address any complications and recommend a course of action.

There are a lot of decisions to make about your implants

One lengthy discussion that happens during your consultation is about your implants and the many different factors that affect your results. This includes things like your incision location, the implant placement, and the implant’s type, size, and profile. It can be overwhelming, so your plastic surgeon can guide you to the right implant and surgical plan for you. Keep in mind that they’re the expert, so be sure to have a good idea of what factors are most important to you— and your intended results, of course.

Your immediate results are not your final results

For the few weeks or so after your surgery, you’ll have to make do with swelling and bruising which can make your breasts look less-than-ideal. It’s important to keep in mind that patience is key and that your final results will eventually settle in as swelling goes away and gravity lowers the implant. Keep in mind that many patients experience some emotional hardship during this stage as they wait and recover, so be sure to talk with your plastic surgeon about any concerns you have.

Breast implant revision surgery often involves the removal of silicone or saline implants for their replacement or to correct complications from your original procedure. When you want to improve your breast shape and appearance after a previous augmentation, breast implant revision surgery at Cosmetic Surgery Associates in Bethesda, MD may help you achieve your goals. Here are some things to know about a breast implant revision surgery with one of our board certified plastic surgeons.

Will breast implant revision achieve my appearance goals?

A breast implant revision surgery is right for you if you do not like the appearance of your previously augmented breasts, particularly after a lengthy period of time since your original procedure. Reasons that women seek breast implant revision include:

  • Deflated saline implants
  • Leaking silicone implants
  • Change in breast or implant size
  • Capsular contracture or shifted implants
  • Stretched skin after weight changes
  • Want to obtain a smaller or larger breast size

Your board certified plastic surgeon is your best resource for knowing if a breast implant revision surgery suits your treatment goals. They can also help you select the right implants for you if you are looking to change them.

Pros and Cons of Breast Implant Revision

Most surgical procedures have benefits and drawbacks. The same is true of breast implant revision surgery. Some of the benefits of this surgery include:

  • It provides more youthful looking breasts
  • You can choose an increase or decrease in your breast implant size
  • Your breast symmetry improves

Cons of breast revision surgery include:

  • Aging and gravity will still change the shape and size of your breasts over time
  • Implant weight affects breast appearance

A breast implant revision does not prevent eventual breast changes, such as shape, size, and appearance. But, revision certainly results in improved appearance from your previous breast augmentation. Your Cosmetic Surgery Associates board certified plastic surgeon in Bethesda can help you understand what to expect from your surgery.

Is this surgery right for me?

No matter how successful your breast implant surgery, you can still experience changes to how your breasts look and feel. This is the nature of aging and gravity. But, undergoing a breast implant revision surgery helps you renew and refresh your look, taking you to a more youthful and symmetrical appearance. Women typically qualify for breast implant revision surgery when they are in good health, do not smoke, and want to change the size or symmetry of their breasts. If you wish to fix problems that occurred after your original implant procedure, such as issues with your implants themselves or the surrounding tissues, breast implant revision is a good option.

Other reasons for wanting breast revision include changes after pregnancy or breastfeeding, weight loss, weight gain, or for a breast lift. If you suffered aesthetic problems after your first procedure, a revision can help you. You can also choose to have your implants permanently removed. This procedure requires you to be in good overall health with realistic expectations and a positive attitude. Your plastic surgeon at Cosmetic Surgery Associates can provide a consultation to help you understand whether the procedure is right for you.

Schedule a Consultation

Breast implant revision can be an excellent option if you’re looking for a change. To get started on planning your revision surgery with our board certified plastic surgeons, contact our office by calling or filling out our online form.

About 300,000 American women undergo breast augmentation each year. If you are one of them, it’s common to wonder about the possibility of breastfeeding after getting breast implants. There are studies that suggest that you may face more challenges while breastfeeding, but that is very rare and most women are able to breastfeed just fine after breast augmentation. Here’s some more information about what to expect.

How Can Implants Affect Breastfeeding?

While implants themselves don’t affect breastfeeding, some factors about the surgery itself can affect it. Here are a few things to consider:

Loss of Feeling in the Nipples

Because of the nature of the procedure, women sometimes lose some sensation in the nipples after breast augmentation. This can make breastfeeding more difficult, depending on the amount of sensation lost.

Implant Placement

If your breast implants are placed under the pectoral muscle, it sometimes proves better for breastfeeding. Although implants don’t necessarily interfere with milk production, they sometimes can if they are placed above the chest muscles.

Natural Breast Size

If you opt for implants that are considerably larger than your natural breast size, then it can sometimes interfere with milk production because of insufficient breast tissue. Your plastic surgeon can give you the best idea of what to expect with your chosen implants.

If your implants contain silicone, you should note that there is no risk to your baby while breastfeeding. Breast implants have been studied at-length for many years, and your plastic surgeon can answer any questions or concerns you might have about their safety.

Schedule a Consultation

Breast augmentation is one of the most popular cosmetic procedures and has a long history of safety and effectiveness. To begin scheduling your consultation and discuss your plans for breastfeeding after implants, contact our office by calling or filling out our online form.

Breast implant cosmetic surgery involves increasing the size of the breast by using artificial implants. It is also known by the name of breast augmentation surgery which closely refers to its cosmetic appeal. It could be used for breast reconstruction in patients suffering from breast cancer.

There are two different types of breast implants. A saline implant consists of a capsule filled with saline solution (salt water). A silicone implant is one in which the capsule is filled with silicone or plastic gel. In the case of silicone implants, the capsule is pre-filled to a standard size by the manufacturers before it is supplied for surgical use. Saline implants are filled during cosmetic surgery by using a small incision. The solution is filled in a controlled manner until the desired shape is formed.

There are advantages and disadvantages to both types of breast implants. Silicone implants are known for their natural feel, but a rupture can be hard to identify. The shape of saline implants can be controlled during the surgery, but irregularities after a rupture will be more pronounced. When saline implants rupture, the solution is naturally absorbed by the body without any side effects.

More About Breast Surgery

Recovery and risk related to breast implant cosmetic surgery

Pain, uneasiness and other minor risks are common to both types of implants. Saline implants will become flattened after a rupture as the saline solution quickly leaks out of the capsule and gets absorbed into the body. Irregularities in breast shape will be drastic in such conditions. Besides, saline implants don’t feel comfortable or natural for most patients.

Ruptured silicone implants do not cause excessive leakage as the hard plastic gel will remain in the capsule for a long time even after rupture. It is hard to detect a rupture in silicone implants. They may pose serious complications if left unattended for a long time. For safety, ruptured implants must be immediately removed or replaced. The absence of conclusive evidence caused silicone implants to be banned for a decade, but further scientific studies and research have proven that both silicone and saline implants do not pose serious damage to the body. It is essential to follow the advice and instructions of the plastic surgeon to recover safely from the surgery.

Safety with implants is further enhanced by imposing certain requirements for patients to qualify for breast implant surgery. In cosmetic augmentation, saline implants are allowed only for patients above the age of 18 and silicone implants for those above 22. In reconstruction surgery, both types of implants are allowed for patients of all ages.

Fat Grafting

Most patients who undergo breast augmentation fear that they will be the next victim of rippling. If you are looking at getting breast enhancement surgery done, then it’s important that you educate yourself about rippling and what causes it instead of fearing it. One of the primary reasons for rippling to occur is that the patient doesn’t have enough breast tissue to accommodate the new implant. Another and not so uncommon reason is that the size of the implant is too big for the patient.

ASAPS Video

Should I  say no to breast implants?

Dr. Franklin D. Richards who is a cosmetic surgeon and works out of Bethesda, MD and McLean, VA; Northern Virginia; Fairfax, VA; Alexandria, VA, Chevy Chase, MD; Rockville, MD, suggests that patients should explore their options before completely closing the door on breast augmentation. Even if rippling occurs patients should know that it is correctable. Unfortunately, it happened but it is not something that you have to live with forever.

How does rippling feel and what causes it?

If you can trace the outline of your implant and feel its texture, it is most probably rippling. Post breast augmentation surgery patient’s body requires time to heal. It is during the healing process that a shell is formed along the newly inserted implants. The plastic surgeon automatically leaves space to accommodate the newly formed scar tissue. It is the path of normal healing and everyone undergoing breast augmentation experiences it. If the scar tissue isn’t formed at all or it is deformed it can lead to rippling.

How is it fixed?

Dr. A. Dean Jabs who is a plastic surgeon and works out of Bethesda, MD and McLean, VA; Northern Virginia; Fairfax, VA; Alexandria, VA, Chevy Chase, MD; Rockville, MD wants to assure you that rippling can be controlled.  It will need surgery in which the cosmetic surgeon can position the implants slightly differently. Patients can also benefit from procedures like Fat Grafting and avoid the surgery altogether.

Is fat grafting safe?

Fat Grafting is increasingly becoming popular with patients as it is a minimally invasive procedure. It allows patients to use their fat cells to fill out the area that’s left empty in the absence of scar tissue formation. The fat is gathered through liposuction from areas like the hips, waist, or stomach. The accumulated fat undergoes various purification processes to remove impurities that initial extraction may have caused. If you are underweight fat Grafting may not be possible for you as the patient needs fat reserves for normal body functioning too. Once the concentrated pure fat is retrieved after the purification process, it is injected in small quantities all around the breast mound to cushion the implants and to even the surface of the breasts. The recovery period post-fat grafting is usually short as it is performed on outpatients. There is no risk involved in fat grafting as long as your doctor is monitoring it.

Thousands of breast augmentation cosmetic surgery procedures are performed in the U.S. every year with outstanding results. The introduction of highly cohesive silicone gel implants, which have been approved by the FDA, and cutting-edge surgical techniques have contributed to the rising popularity of breast implant procedures in the country. However, the procedure is complex, and it takes an experienced breast surgeon to ensure safe and desirable results.

Board-certified plastic surgeons, Dr. A. Dean Jabs and Dr. Franklin Dr. Richards of the Cosmetic Surgery Associates provide advanced surgical procedures for the breast, body, and face, including breast augmentation surgery. Both surgeons command a combined experience of over four decades between them. Operating out of offices in McLean VA and Bethesda MD, Dr. Jabs and Dr. Richards receive patients from Northern Virginia, Fairfax, Alexandria, VA, Chevy Chase, Rockville, MD, and surrounding communities.

He will help the patient make the right choice of breast implants in terms of their type, size, profile, shape, and texture. He will involve the patient while making optimal decisions about the type of incisions and the placement position of implants in the breast tissue. For his personalized approach, Dr. Richards enjoys a strong reputation among his patients for breast implant surgery and other procedures.

In addition to his private practice, Dr. Richards also currently serves as the Chief of Plastic Surgery at Suburban Hospital in Bethesda, MD. In 2002, he was chosen as the President of the National Capital Society of Plastic Surgeons.
